The C-wire adapter doesn’t get installed behind the thermostat or require you to run any new wires through the wall. It installs at the HVAC equipment (air handler or furnace) and repurposes the existing thermostat cable.

The adapter works by electronically combining and separating signals so the thermostat can get a dedicated C (common) connection while still using the same bundle of wires that’s already in the wall. At the equipment, the adapter connects to the control board and the existing thermostat wires. At the thermostat, you simply reconnect those same existing wires according to the adapter’s instructions.

So, in a typical installation, no new thermostat wire needs to be fished through the walls. At the thermostat, one of the existing wires will be moved to the C terminal based on the adapter’s wiring instructions. You’re not adding another conductor. The adapter is electronically creating the C-wire connection using the wires that are already in the wall.
